If you have difficulty filling these out, please call the office and we will do it for you.
- We will text you from our company phone number and request that you respond to the email with a picture of your driver’s license and valid medical marijuana card.
On the day of your appointment please make sure you are available for approximately 30 minutes.

Submitting State Application
Expedited Option: We encourage all patients to take advantage of our Expedited online application submission to the State. Not only does this expedite the process of you receiving your card, but it eliminates the transferring of any paper documents, it is more secure, and you receive same day confirmation/temp. license from the State of Michigan via email that can be used upon receipt. The fee is $25.00. If you are assigning a caregiver you can NOT use the expedited option.
Do it yourself (DIY) option: If you prefer to mail the paper application, we will mail the original documents to your home and you will be required all state fees and complete the application on the Michigan state registry website.
